Credit for a Failed Internet Certificate of Status Transaction

To issue a credit for a failed Internet Certificate of Status transaction the following information must be submitted by fax to 410-333-7253.

  • Date of the transaction.
  • SDAT department ID number (e.g. D00000000).
  • Name of the business entity.
  • Information for the credit card used during the transaction.
  • Your name and daytime telephone number.

You may use the form below to send in your request. This form is to be used only for failed Internet Certificate of Status transactions.
